Primary Care Services
Our medical staff is available four days a week. Primary care provides a wide array of services including; physicals, health promotion and maintenance, diagnosis and treatment of acute and chronic illness and patient education. The Lower Shore Pharmacy (not affiliated) is also onsite for your convenience.

Outpatient Mental Health Services
Individuals in need of services are assessed by clinical staff and also receive a psychiatric evaluation which includes history, mental status examination, and diagnosis. No one is turned away for the inability to pay.

Addictions Treatment
HealthPort is committed to assisting in filling the gaps of treatment and recovery for individuals who suffer from co-occurring disorders, substance use, mental illness, and gambling addictions. HealthPort offers evidence-based interventions such as Medication Assisted Treatment (MAT) along with positive psychotherapy and neurofeedback training.

Psychiatric Rehabilitation Services
Onsite Services
Day program services are available seven days a week in Wicomico, Worcester, and Somerset counties. The program includes groups pertaining to mental health issues, daily living skills, physical health education, and many more activities varying each month. We provide breakfast and lunch.
Community Outreach Services
Psychiatric Rehabilitation Specialists assist with the daily demands of life and provide services in the community, home, worksite or another location chosen by the member. These services include, but are not limited to: money management, gaining and maintaining entitlements, acquiring and maintaining employment and housing, connecting to community resources, interpersonal and family problems and maintaining good overall mental health.
The goal of the program is to provide services to assist individuals with developing and maintaining the highest level of independence as possible.

Residential Rehabilitation and Supported Housing
Our goal is to provide safe and affordable housing to adults with severe and persistent mental illness and to assist individuals with developing and maintaining the highest level of independence as possible. We currently provide supervised, unsupervised, and supportive housing. The organization also provides Supported Housing to members and the organization acts as a landlord and rents apartments/rooms to members.
Staff provides varied levels of support to assist the members in achieving goals including but not limited to; budgeting, shopping, meal preparation, treatment planning, and medication monitoring. All supervised housing utilizes digital technology to administer medication.
Residential services are available in Wicomico, Worcester, and Somerset counties.
Employment Support
HealthPort provides vocational rehabilitation services in Wicomico, Worcester, and Somerset counties. The program assists adults with mental illness to develop, attain, and maintain a job in the community.
Our vocational services are designed to be flexible for the client, so that they may access assistance from the vocational staff at any time. This includes periods of employment and unemployment.
We provide ongoing assessment and planning with the client, and when appropriate and with consent, includes family or natural supports.
We partner with the Division of Rehabilitation Services and Maryland Employment Network to assist individuals to obtain employment and to work toward economic self-sufficiency.

Assertive Community Treatment
HealthPort’s Assertive Community Treatment Teams (ACT) provide mobile psychiatric, employment, rehabilitation, nursing, peer support, housing, and substance use services. ACT teams provide outreach at a client’s home, work, or any other appropriate location chosen by the client. The teams provide case management and advocacy to assist clients in becoming as independent as possible. Adults in need of services are assessed by clinical staff to determine the client’s needs, strengths, resources and also receive a psychiatric evaluation which includes history, mental status examination, and diagnosis.

Residential Crisis Services – The Retreat
Take a tour of the Retreat!
The Retreat offers a safe harbor and sanctuary for adults experiencing a psychiatric crisis. The Retreat provides short term Residential Crisis Services in a community-based setting 24 hours a day, 365 days of the year, as a voluntary alternative to hospitalization or to shorten the length of a hospital admission.
Anyone who is medically stable, under the care of a physician, awaiting admission to or stepping down from a psychiatric unit can be safely cared for at The Retreat.
Situated in a quiet community, The Retreat’s trained crisis staff offer the comforts of a home-like setting augmented with counseling and therapeutic groups. The Retreat will assist you with meetings with mental health providers to further enhance your recovery. Our private accommodations support clients’ need for calm and confidentiality.
Food is Medicine Program
The Food is Medicine Program is in full swing at HealthPort. The purpose of the Food is Medicine Program is to increase member access to fresh produce, provide nutritional education, teach cooking skills and positively impact their health, all while eliminating food waste in our community. We provide whole food meals that do not have any ultra processed foods nor any additives, chemicals, or preservatives.
Personalized Health Services
Our goal is to improve the overall wellness of a person by assisting them with medical and social issues along with their mental health needs. Our teams help members manage chronic medical illnesses. Staff will coordinate a member’s medical care, provide education and take them to appointments when necessary.
Senior Citizen Services
Senior Services provide high quality mental health rehabilitation services to people over the age of 55. Our goals are to advocate for and encourage seniors to develop self sufficiency, interdependence, and maximized potential while maintaining good physical and mental health.
Senior Services are a resource designed to provide seniors with services needed to enhance their quality of life and make independent living a viable option

Young Adult Services
This program serves young adults from ages 18-29.
Our focus is to provide skills training and a sense of belonging to young adults with mental illnesses to help them acquire the skills needed to live integrated lives within the community. These vital life skills are the cornerstone to lifelong recovery. With this, clients are able to return to the community and live productive lives.

SOAR (SSI/SSDI Benefits)
SSI/SSDI Outreach, Access, and Recovery (SOAR) is a program designed to increase access to disability income benefit programs administered by the Social Security Administration (SSA) for eligible adults who are experiencing or at risk of homelessness and have a serious mental illness, medical impairment, and/or a co-occurring substance use disorder. HealthPort is the geographic lead designated by the Maryland Behavioral Health Administration.
